Adam Hardy

Adam has more than 20 years of experience in nonprofit and technical strategic leadership. At Future State, he supports community initiatives across the United State that focus on integrating and analyzing community data to support coordinated efforts aimed at improving place-based conditions. The organization has collaborated on projects such as the Community Information Exchange in Fresno, CA, Integrated Developmental Screening in Marin County, CA, Coordinated and Integrated Data System for the Early Identification (CIDSEI) with the Center for Disease Control, Strategic Early-childhood Systems Development with the Texas Department of State Health Services, and has recently taken a lead role in developing a National Data & Technology Hub for Place-based Partnerships with Ballmer Group.

Before founding Future State, Adam served as the inaugural Executive Director of Achieve Brown County, where he led a cradle-to-career collective impact initiative. He also played a key role in developing Brown County’s first Community Information System during his tenure at Brown County United Way.

Chris Elfner

Chris brings extensive experience as a healthcare leader, strategist, and entrepreneur focused on advancing data integration to improve social determinants of health (SDOH) outcomes. He brings deep experience aligning healthcare systems, payers, and community organizations to support value-based care models that use integrated data to drive equitable, person-centered outcomes. As a strategic coach and consultant, Christopher has worked across sectors to design and implement initiatives that leverage shared data to improve care coordination, resource allocation, and policy alignment. His expertise spans from strategic planning through execution, with a strong emphasis on operationalizing insights from multisector data. Chris supports teams in designing clear, measurable success metrics that scale with innovative solutions which close gaps in care and improve quality of life.

Kent Maxwell

Kent is a data systems expert with a strong focus on building infrastructure that enables cross-sector data integration to advance outcomes for place-based partnerships. With over 25 years of experience in business and information systems, Kent specializes in designing and implementing modern, cloud-native architectures – such as lakehouse environments – that support secure, scalable, and interoperable data solutions. He transforms complex datasets into actionable insights for leadership teams, driving data-informed decisions that improve equity and outcomes across communities. Kent’s technical strengths include Azure Databricks, Snowflake, Python, SQL, and Bash, with deep expertise in data sourcing from databases, files, and APIs. His work bridges data architecture, engineering, and analytics to power shared measurement, population health strategies, and real-time reporting across health and human service systems.

Adam Carney

Adam Carney is a data analytics and integration specialist with over a decade of experience designing and implementing solutions that advance positive outcomes. With a strong foundation in data modeling, ETL development, and systems integration, Adam excels at translating complex data into actionable insights that support cross-sector collaboration. He brings deep technical expertise across platforms including Oracle SQL, PostgreSQL, Python, Tableau, and Alteryx, and has led agile teams in developing API-driven, interoperable solutions that connect health, human services, and education systems. Adam is a skilled project leader and relationship builder, known for fostering inclusive team environments while consistently exceeding performance targets. His work enables data-informed decision-making and supports sustainable improvements in care coordination, equity, and community wellbeing.
